A Day in the Life: Care Management

Welcome to our blog!

Since this is our first article, we’ll address the service most often requested from AdvancedRM, case management, often referred to as care management.

We believe everyone is entitled to access to the best possible care and all resources available. The job of a care manager is to get to know you and understand your problems. Your care manager will analyze the barriers prohibiting you from getting proper treatment and services, and find ways to overcome them.  They will dig for answers and resources.  They ask the questions you don’t know to ask. They find the answers you don’t know exist.

A day in the life…

Care managers essentially assume the role of “Captain of the Ship”.  The “Captain” brings all of the moving parts together by linking resources, family members and friends, providers, and community programs to effect a positive change. The care manager gets to know each client’s individual situation, clearly understands the social and medical background, identifies problems and goals, and then finds ways to reach them. They maintain close contact with their clients, advocate for them, and get results!
